Our tutoring program is proud to showcase student learning, engagement, and success. We occasionally post photos or short video clips of our students participating in tutoring activities on our official company website and social media pages.

Parent/Guardian Consent

I GIVE permission for my child’s photograph, video, or likeness to be used by Polk Clark (GCTS) for promotional or educational purposes on the company’s website, social media, and printed materials.

I DO NOT give permission for my child’s photograph, video, or likeness to be used by Polk Clark (GCTS)

I understand that:
-No personal identifying information (such as last names, addresses, or contact details) will be shared publicly.
- I will not receive compensation for the use of these images or recordings.
- I may withdraw this consent at any time by providing written notice to Polk Clark
